That is one ugly mouth. I would extract the lower incisors and bridge in some smaller ones. I think I would reshape the maxillary laterals to make them a little shorter.

Do they have to be so opaque. I'd like to see some translucent highlights on the incisol edges.

I don't see any need for Ortho unless you want to add a little labial Crown Torque. Yea... that'd be nice. Does the patient have deep pockets? Good insurance? If she does.... Please tell her I think this looks like an Orthodontic Emergency.
