CNC losing home

Hey guys,
We have a Vision 1624 CNC and a Series 3 controller. Today the machine has been starting a job correctly but after a few minutes it shifts the rest of the job over onto what's already been cut. When I stop the job and send it home it goes about halfway down the X axis instead of 0 on both X and Y so its getting it's origin messed up half way through a job.
After restarting the controller it appears to have home reset correctly but then the problem repeats itself once a job is underway again.
Anyone have any suggestions?

Sounds like lost steps during the load. Guessing loose/slipping couplers at the screw or motor mount. It is mechanical and not software error.
