    That's a cup hinge designed to fasten to the front of a face frame. I don't recognize the maker. You may be able to replace it with a Blum Compact 33, using the mounting plate for fastening to the front of a face frame.

    Looks similar to a Mepla, probably discontinued. Grass has a very similar one(currently).
