Jessem 06001 Mite-R-Slide & Mast-R-Slide both New/Sealed-San Jose, CA
I have the Jessem Mast-R-Slide:

Tried PM twice.. let's try it this way
 Any idea how much to ship the Mast R Slide to 01748 Hopkinton MA? Meanwhile I am trying to figure out if I can put it on my SawStop... not much info either way that I can find. Had the accessory on my PM66 and loved it... but had to sell with saw to afford Sawstop.. now am misisng it!

Just got back in to town. I am contacting ppl in the order of their response.
