John, Try the beisemeyer outfeed table. It's 4' x4' with slick high pressure laminate. You will also need legs. Amazon carries them, Kinda steep though, cost you about $300 for both. Or you can buy the legs for about $50 and build the table yourself, although I believe the table comes with mounting hardware
Jim Becker's is very nice and as he noted popular with many having built similar ones.

I bought the Beidemeyer legs and built the table myself. 4'x4' is enough to keep a sheet of plywood from falling off.
John, I was one of the people who used Jim Becker's design and absolutely love it. I added it to a contractor table saw and it should be no problem adapting yours to the design. You wanted pics, you have pics of mine below.
There's one in every crowd......and it's usually me!
