I have a cheap 4" angle grinder with a 40 Grit disk on it. It will eat 4 x 4's, nails, screws and everything else in its path. It come in really handy for that sort of thing. When it comes to protrusions, it is my 'go to' tool.
In really tight areas, I use my Dremmel with a steel cutting disk.

    Dremel,angle grinder, or belt sander.

If you want to keep the surface of the wood as flat as possible, I would suggest clipping the nail or screw tip as much as possible with a set of diagonal cutters, and follow up with a flat file.
